If you are interested in becoming a member of the Derby City Disc Dogs, you'll just need to...

  • Download, print, and fill out a membership application
  • Attach a copy of the rabies certificate(s) for each of your dogs.
  • Mail the application, along with applicable membership dues, to the address listed on the application.
